Click this link to open a PDF form you can fill out to let us know which plate size you want, pick one of the 8 font style choices, and tell us what you want written on it. The large engraved plate looks best with a maximum of 25 characters, and the small plate maxes out with two lines of 20.

Afterward, you will have the option to e-mail the form (please choose the option to send as AN ATTACHMENT to include the information you filled in), or print it and mail it to us with your payment. If you prefer to pay online, return to our home page and use the "Engraved Plate" menu item (last one in at the bottom left) to use the shopping cart to pay for the plate. Cost for either size is $9.95, which includes shipping and handling.
